Provider-to-Provider Referrals

To refer a client to our Santa Fe facility: Call 505-471-4985 Monday-Friday between 8am-5pm.

To refer a client to our Gallup facility: Call 505-670-0774 Monday-Sunday between 8am-10pm.

Please provide:

• ER/hospital progress notes and discharge summary
• Current medication list
• Medical records pertaining to chronic conditions
• Psychiatric notes
